All scheduling is done online.  After your first appointment, you will have access to Dr. Lott’s appointment calendar.  You will be able to see what time slots are available to best meet your needs.  You can also cancel online; however, there is a $90 fee if you cancel with less than 24 hours notice. 

 

New Patients

Dr. Lott only accepts new patients who are between the ages of 6 and 12.  

 

To schedule with Dr. Lott, you will need to follow the link below where you will be given the option to register as a new patient/client.  You will then be asked to complete the necessary forms with contact information, insurance and other background information.  You will be asked to create a simple login and password combination so you can access the schedule in the future.  When all the forms are completed Dr. Lott will be notified and he will call you to schedule your first appointment. 

 

When you click on the link below, you will also have the option to view the availability of openings in Dr. Lott’s schedule.  This will give you a realistic expectation on how soon you can be scheduled. 

 

For the first appointment Dr. Lott would like to meet with the parents/guardians of the child alone.  So please schedule when your child is in school or with a sitter.

 

Returning Patients

If you have already registered and have your login and password, simply follow the link below to check when your next appointment is scheduled.  You can add or remove appointments as needed.  If you fail to show for an appointment, you will automatically be charged $90.  If you cancel with at least 24 hour notice there is no charge.  If you cancel with less than 24 hours notice, you will be charged $90.00  Extreme circumstances can be discussed with Dr. Lott.
